Arizona Community Help, works to ensure that all eligible households receive the maximum level of benefits and assistance available. It is our goal to provide you with resources or direct assistance within 72 hours. In order to do so, we ask that you complete the simplified application below for all members of your household. Please upload all requested documentation so your application can be immediately reviewed for approval and assignment. Failure to fully complete the form or requested documents will result in delay or decline of available benefits.
